  1. This form is a request. Your appointment is confirmed only when the clinic confirms it by phone, WhatsApp or email.
  2. The time you choose is a preference. If it is not available, the clinic will offer the nearest free slot.
  3. Please arrive 10 minutes early and bring any previous prescriptions, reports and a photo ID.
  4. To cancel or change your appointment, please let the clinic know as early as possible.
  5. Consultation and treatment charges are as per the clinic's current fees and are payable at the clinic.
  6. This form is not for emergencies. In an emergency, go to the nearest hospital or call 108.
  7. Please make sure the details you give are correct. The clinic is not responsible for delays caused by incorrect contact details.
